When things go wrong in the workplace it is important to understand why. A business must aim to prevent a reoccurrence. Human Safety Systems specialise in undertaking incident reviews to understand what went wrong. The findings assist management and the entire organisation learn from a negative performance indicator.

Human Safety Systems has a strong focus on human performance in safety operations. Incidents are often caused by human error. Human Safety Systems brings an additional level to the table for understanding why a person undertook a particular action. We don't believe in a blame culture for the workplace, and don't assist businesses along these lines. Instead, Human Safety Systems can assist if you aim to identify all critical and casual factors relating to an incident and want to prevent it recurring.

For an incident caused by human error, we emphasise understanding why the incorrect action was taken while focusing on better courses of action to take in future. This is often referred to as 'active failure'. Other factors in the workplace, called 'latent conditions', can contribute and are also addressed.
